Tattoo Request Form
Interested in getting a tattoo from me? Fill out the form and I will be in touch shortly. This just gives me a general idea of what type of service you are interested in. I will then be able to determine what type of appointment is needed to move forward. For example if you are wanting a small tattoo we would most likely go right to scheduling the small tattoo appointment, but if you were wanting to start a larger project, I would have you book an in person consultation first and then go from there. Some tattoos require consultations and some do not, this form is the easiest way to go about scheduling and managing everyone’s time. Thank you!! - Mike
